How to Book a Session

The easiest way to book an appointment is to click below.

Getting Started

Once you have requested a session, you will be sent intake paperwork via email. Please review and sign the informed consent, practice policies, notice of privacy practices, demographic information, and the intake questionnaire. The paperwork must be completed prior to your appointment.

No Show/Cancellation policy

There is a 24 hour cancellation and reschedule policy. After 24 hours, you will be charged $50 for your scheduled appointment. It is to Guided Transformation’s discretion to forfeit incurred charges. $125 will be charged for missed/no-show appointments for new and reoccurring clients.

After 10 minutes of being tardy, you are still responsible for the full fee and the session may not proceed and you may be asked to reschedule.

Your insurance does not cover late cancellation and missed appointment fees. You will be solely responsible for these fees.

Please understand that if you are using insurance, the service must be rendered a medical necessity which means a diagnosis must be present. It also limits confidentiality between you and the therapist due to sharing of information for service reimbursement. If this is a concern for you, please ask about your payment options at the time of service.

We do provide a Superbill to obtain service reimbursement from your insurance company. It is suggested that you call your insurance provider to find out what Out-Of-Network fees your plan offers. Receipts will be automatically sent electronically after each session and superbills at the end each month.


Sliding scales are to the company’s discretion and may be granted to clients with financial restrictions.
